The parish of the Annunciation of the Blessed Virgin Mary in Tomaszów Lubelski was erected on May 6, 1674. At that time it belonged to the Chelm diocese, Potylice deanery. The parish church of the Annunciation of the Blessed Virgin Mary was founded by Ordynat Tomasz Zamoyski (1627-29), who also endowed the newly erected parish. At the time of the parish's erection, there was a branch church of the Holy Cross in Tomaszów, belonging to the parish in Chodywańce. The church dated from 1610-14, and was located in the Lviv Suburb. It was still mentioned in the bishop's visitation of 1774.
Due to the poor condition of the first parish temple, a new one - founded by Ordynat Michał Zdzisław Zamoyski - was erected in 1727. It has survived to the present day and is known as a larch church-sanctuary.
Built in 1727, the larch church is one of the best-known wooden Baroque churches in Poland and the only two-century church in the Zamojskie region, an example of the full application of Baroque forms in wood. The structural solutions of the interior prove that it was designed by a seasoned architect associated with the Zamoyski family.
The Church of the Annunciation of the Blessed Virgin Mary performed parish functions, except for the years 1955-87, when the headquarters of the parish was the Tomaszow shrine of the Sacred Heart of Jesus. The new administrative division (October 25, 1987) established three parishes in Tomaszów: of the Annunciation of the Blessed Virgin Mary, of the Sacred Heart of Jesus and of St. Joseph.
